Primary medical care aims to provide an entry point into the health care system. As a medical home provider, we offer routine, preventative, and chronic care management that is continuous for individuals and families of all ages. We coordinate other healthcare services as they relate to an individual’s care. Services are provided by Physicians Nurse Practitioners and Physician Assistants who comprise a team of medical providers in family practice, internal medicine, pediatrics, women’s health, and geriatrics.
We participate in all Medicare ‘D’ prescription drug plans. Most insurances are accepted.
