Hours: Foundations Pediatric Therapy, LLC. (FPT) is an outpatient therapy clinic in Dothan, AL that offers pediatric occupational, physical, and speech therapy services. Formerly known as Children's Rehab and Therapy Services, FPT was opened in September 2019 by Jake and Kristin Helms.
