I am disabled and requested accommodation in accordance with Title III of the ADA. I was unlawfully discriminated against and denied ready, full, equal, independent and dignified access to and the opportunity to benefit from your services.
The ADA requires accommodation or modifications, and if denied, must be accompanied by a WRITTEN explanation (by somebody in a position of authority with both spending, making a decision making authority) DEMONSTRATING how the request FUNDAMENTALLY alters the nature of the service, program or activity, and must be accompanied by EFFECTIVE ALTERNATIVES, so that’s the maximum extent possible, I can participate in and enjoy the benefits of your service.
Additionally, when checking in, it’s confusing. There is no one there to greet you or to answer any questions. If you have any disabilities like blindness or difficulty reading, or understanding, you’re stuck and out of luck. there needs to be either a person or a Audible messages that tells an individual or guides an individual with vision disabilities where to check in and how to check in. Additionally, if somebody has a learning disability or cognitive disabilities, how are they to check in if they can’t figure it out or they can’t read the check in Screens?
Next, there is no person to interact with, so how would one request an accommodation from an inanimate check in terminal? You couldn’t! That is not ADA compliant and violates the ADA.
Next, the check in screen that says or advises that a copy of your ID will be copied and MAINTAINED on file is threatening, coercive, intimidating, interferes with one’s rights and violates the ADA, as well as the DPPA. There needs to be a notification or notice to the consumer that you would NOT be required to do so, and declining to allow your business to copy The ID would not interfere with your ability to participate in and benefit from their services; you would still be seen.
I will be reporting this to the department of justice and contacting an attorney. Noncompliance with the ADA is inexcusable and failure to receive technical assistance from the department of justice does not relieve you of your obligations or liability.
This also involves FDUTPA, the Florida Civil Rights Act, and Tort Law; negligent and intentional infliction of emotional distress. I was mortified and irreparably harmed by your business.
Folks know your rights and stop blindly following directives of businesses. Stop allowing businesses to copy and maintain copies of your drivers license. It is not required by law. It cannot be required by law. They only need to verify your identity to ensure that you are the person come in this scenario, that is using the insurance.
Folks exercise your rights under the Americans with disabilities act. See 42 USC Subpart II which applies to governments and Subpart III which applies to businesses. See also 28 CFR 35 for governments duties and your rights and 28 CFR 36 for businesses duties and your rights.

Lady with long black hair made my daughter stand in crutches for over 10 minutes and did a terrible job withdrawing blood resulting in me getting hurt.
On Monday morning (March 10 2025) I assisted a programes appointment in quest diagnostics in Doyle. I was attended by a young woman with long straight black hair who stated to be Colombian and whom speaks Spanish. She spoke in a rude tone to me because I requested a chair for my 12 year old daughter which found herself in crutches, having a cast in her right foot, making her stand for over 10 minutes whilst she took care of me. When she attended me she allegedly could not find any veins in my arms. Attempting to do so in my right hand despite me asking her not to because it hurt a lot. She insisted that it was going to be there where she would withdraw blood. She ended up doing movements with the needle that produced a ton of pain, to the point where I started sweating excessively from pain. She ended up making me cry afterwards. That’s when she called another woman to come attend me. The Second Lady: curly dark hair, managed to withdraw my blood from my left arm in the first attempt. It’s important to have empathy with patients, be respectful and also be kind with them. Especially since a lot of us come with already bad health situations. Just to be treated like this.

The nurse who administered the testing was thorough, professional, and delicate. 10 out 10 had a pleasant experience. The facility was clean, and with an appointment scheduled, the wait time was minimal after check in.

Waiting around really not good. Came in as walk in at 9:30. Screen said 1st available was 11:40. Asked if I wanted to wait inside. I said yes, thinking I'd be in when they could get to me. Instead I watch the waiting room go empty several times and never got called had to wait the entire 2+ hours. The screen should have said come back 15 minutes early and not have me wait, if they were never gonna even try to get me in while waiting.

Today I went to have a blood test ordered by my primary doctor, and it was a complete disappointment. I had scheduled the appointment through the Internet and it was for today, November 5 at 8am, and after fasting and not attending the work they tell me that there was no such appointment. They treated me like a black person during the time of slavery, I did not imagine health workers being so dehumanized and lacking empathy towards their neighbors.

Don't come here if you can avoid it. The people here are very nasty and rude and don't help you in any situation. They kicked me out of line due to their issues and when I was able to get them the paperwork they should have already had they made me try and wait a few hours even tho I had an appt. I have never been to a quest with this amount of rudeness. Give them some training

My tech was exceptional .. I left my paper wok at home .. live only 10 mins away could not turn in my
My Red Urine bottle with out my paper work...She allowed me to sit in the waiting room .. while my husband left to go fetch my paper work for my 24 hr Urine sample. As she went on to do my blood work.
He returned with the paper work which I was happy 😊.. I was Not looking forward to having to repeat that 24 hr collection again...

My newborn preemie needed blood work for an upcoming appt. I was given paperwork for bloodwork when my newborn was released from his 4 week stay in the NICU. Came to Doyle location. Two employees were rude and not helpful. After coming from a different location because they ran out of butterfly needles, this location claims my paperwork I got from the hospital had no usable info on it. So I called Winnie Palmer Hospital and had the NICU representative speak to her. Quest employee was so rude to the hospital rep, wouldn’t let her finish speaking. And had clear annoyance with the situation. Once they came to a conclusion I was told it takes over an hour for a fax to come thru and I had to sit in the waiting room for that time with my preemie. Winnie Palmer called me 30 minutes later asking if it all worked out and I told them they still haven’t seen us and she confirmed on her side the fax was completed and shown sent. Employee insisted she had nothing so I had to leave to feed and change my newborn. Very disappointed.

Pretty much what you would expect from a phlebotomy lab. This morning the staff were exceptionally rude complaining out loud about working there before a hurricane that isn't going to affect this area until many hours after their location closes for the day. The signs there that say "heroes work here" need to be taken down because every employee there was openly talking about how mad they were that patients were coming in, infront of patients. Downright embarrassing. Switching to labcorp after this visit.
