This facility is immaculately and incredibly clean. The administration is knowledgeable and committed. The workers are friendly and thorough in their duties. This facility was very cleaned, smelled great throughout, all patients that I seen were cleaned, all staff that I talked with and those that I passed during my tour said "Hello" to me first. This was an enlightening experience considering other Transistional Rehab. Facilities that I've toured.

This place is no 5 star facility but, the staff that cares for my son is beyond 5 star. My son is loved by many. He has an amazing CNA, an amazing roommate and amazing staff that surrounds him. I wish he could be at home with me but, we are both happy about being here.

Bad communication on setting up interviews. I accept a Arrange time and date only to get me dismiss . Without speaking with the Executive Director. They must do better on communicating with applicants. On cancelations before wasting their time . No excuses

I visited during game time,, the patients seemed to enjoy bingo a lot.. The staff was friendly & attentive to the patient's needs, the facility was clean & lunch was well balanced. I'm pleased with my loved one's care & he looked well.

The worst the worstThe worst experience any medical facility that one can imagine some good some good and some bad nurses. One nurse assistant seemed to be a gang leader; enforcing her commands to others with beatings that could be heard through the thin walls. Sexual acts were performed on my roommate with me in the room. The guy next to me in the room died and I wasn’t removed through the attempts to revive him. No counseling offered. Stay away … this place should be closed today!!!!!some

The center has did a complete makeover. The staff is very friendly. They have high tech security that takes your temperature. It also has facial recognition. The patient/client that assisted us was very knowledgeable and helpful in getting us checked in. There was no lingering smell and it was very clen.There is no entry or exit without an employee put in their code to open the door.I would feel completely safe knowing that a friend or family member is there.

The place was very clean,there were no odor lingering in the air nor in my auntie room,she has been very well taken care of ,some of the resident appear to be happy some were friendly and talkative and so was the staff, they were professional, uniforms were clean,great personality, and helpful without attitude, I'm greatful and aprective for this place they really take excellent care of my Gwen,I love them for that!!!!

The facility was very clean and smell very pleasant. It’s nice to know that my Aunt is somewhere that people take care of her if she had a smile on her face with so much excitement.

My dad is a total care he can't talk either and the Foulton Center has taken advantage of that had not did any hygiene care in weeks my dad face was so dirty that the wipe I used to wipe his face was brown he had what look like a stage four wound on his bottom I sent an email and we had a meeting but nothing that was promised was done no hair cut still did not wash hair and form what is going on now there was still no proper wound care now he has ostomenlits he also has a peg tube that they were not given him water so then they had to give him IV fluids but they tried to say his peg tube came out but either way I was never notified about none of it it was by accident that I found out please please if you cant keep them on their toes please dont put your family in this place

The personnel/staff is friendly & helpful. It's clean overall but has an nursing home smell at the same time. Also this facility is in walking distance to the nearest Wal-Mart.

My brother and his roommate are well cared for. I was please with everyone and everything except my brother's hair was Not, washed, cut or comb. I comb it myself. I don't mind. I'm a CNA too. I'll take care of anyone in need. Clean, fresh smelling facility, nice staff, happy residents too.

Roachest on wall and have pee stains on bed. bad smell in hallways when uush button they take all day at night 4 patients too a room tv not on the wall rite cord hanging some beds don’t have lights over there bed food cold and nasty They run out of briefs every day and wipes patients have to share 1 bathroom that’s crazy nurses be talking about what they not going to do patient be falling on floor and they will just put a mat under them and let them stay on the floor also u have to ring call button more then once’s before someone come help u I was there 1 month ago and I won’t go back do not put your family members there
