00000000000000000000000 stars! The care at this place is disgraceful. My mom was sent there to recover from a broken knee. The staff is highly neglectful. 1) Ran out of her pain medication, she was in excruciating pain and had to wait hours for the pharmacy to deliver.2) Several staff members would bang her injured knee when assisting her with using the bathroom.3) Took her to dialysis several hours after she was told she was going which caused her to not to have dinner. Her food had been delivered to the room while she was gone and sat there for a couple of hours. Did they really expect her to eat the cold hamburger that had been sitting there for hours?4) Did not let her shower or give a spounge bath for a whole week.5) For several hours they refused to call 911 at her request when she had a very severe bloody nose that wouldn't stop bleeding.These people should be ashamed of themselves.MM

I had been at Bella Terra Streamwood for two months for physical and occupational rehab. The floor personal (nurses and CNAs) were all skilled, caring, attentive and friendly. The management team were able to address all my questions, concerns and needs. The therapy staff developed custom therapy sessions which worked me hard (which I requested) and I went home with no concerns about whether or not I'd be able to function independently. I love and respect the entire therapy team and would not hesitate to work with them again.

Great Staff... They show they really care, even management are hands on with patient care. I have been to other facilities and management seem to hide in their offices. At Bella Terra the nurses, administrators and therapy all work together to make the team work like a well oiled machine to make sure the patients and residents are taken care of. Thank you to all the staff over at Bella Terra Streamwood

I have been at Bella Terra Streamwood for approximately 1 month for post surgery rehab. All of the team here has been extremely helpful with my therapy. The level of care and concern at all levels from CNA's, Nurses, Rehab Specialists, Doctors on up to top Management has been excellent.I should also add the facilities are very clean and comfortable.

Very nice place and recently redecorated. Staff very friendly with a very good atmosphere which does help with the healing process. Physical therapy staff there nearly every day. I would highly recommend Bella Terra Streamwood.

Check Medicare website. Their stats don't Lie. Focus on Reviews for Short Term Rehabilitation. Their website is misleadin Don't make the mistake we did. Our decision to send Howard to Bella Terra cannot be undone. End of Nov 2022, our 97 yrs old, WW2 Marine was living independent with assistance for showers, medications & meals. Everynight he called his financial institutions to check his account balances. On Dec 10th, he was transferred from CDH to Bella Terra for short term rehabilitation to regain his strength. He took his last breath on December 26th at 8:50pm.Although we are deeply sadden by his death with emotions extremely high, it is important that this review is posted to help validate the other families with similar experiences. Details will be posted at a later date.Our loved one was placed in a room where he was unable to see or hear the TV and left without any mental stimulation except for the room phone. When the phone was placed within his reached, he was able to place a call. The room did NOT have the standard recliner found in most hospital rooms and other rehabilitation centers. He spent numerous days in his hospital bed NOT a chair looking at a TV he couldn't see or hear. This is NOT Short Term Rehabilitation.Medicare requires a Care Plan where the nursing home staff, Physical, Occupational & Speech Therapists will provide a health assessment of current health conditions and present the plan of treatment or goals that the therapists hope to achieve. Bella Terra met the 14 day completion within 9 days but none of the therapists were present during this call & no therapy info provided!Nursing homes are required to submit this information to the federal government. This information is used for quality measures, nursing home payment, and state inspections.When he was first admiited, the social worker contacted us to basically welcome us to Bella Terra. Although my husband has Power of Attorney, our request to be notified for EVERYTHING related to his care was ignored. As POA, we had to submit a fax requesting changes in his diet, 3 separate times to the same fax number over a 24 hour period. Except changing the cover letter and including confirmation of the previous faxes transmitted correctly, it was the same fax.As POA, we submitted a request on December 20th to transfer our loved one back to his studio apartment at Franciscan Court Supportive Living but the private care givers until his needs and care could be determined and Franciscan Courts Nurses, Therapists and staff could resume providing care. Bella Terra Streamwood would not provide the medical tranfer documentation to the Franciscan Court Staff after numerous requests both inperson and via email until December 26th.As POA, we continued to request copies of all his medical records as well as sending the medical transfer documentation to Franciscan Court Supportive Living, the decision was made for Hospice on Dec 22nd. After Traditions Health Hospice Care contacted, they had difficulty obtaining the necessary documentation.As POA, on Dec 24th, feeling desparate to get our loved one out of Bella Terra ASAP, we went to Bella Terra to get the necessary documentation. We ran into Traditions Health Hospice on-call in-take nurse to finish the admissions paperwork. We also met Debbie, the administrative supervisor who we mistakeningly trusted & believed would follow through with ensuring the transfer medical documentation would be sent to Franciscan Court.Several calls were made to Bella Terra Streamwood the morning of December 26th demanding the transfer medical documentation be sent to Franciscan Court which was not sent as of 12pm. Luckily, Gina, Traditions Health Hospice, arrived at Bella Terra around 1pm. Gina was able to accomplish what we were unable to - COOPERATION from Bella Terra Streamwood.The transport ambulance was ordered around 1:30pm with a pickup time of 3:30pm. He was tucked into his bed at his home at Franciscan Courts by 5pm and listening to Shark Tank by 6pm. His last breath was at 8:50pmBella Terra ig

My name is Daniel Schwartz and I am the current business development manager at Home Instead in Itasca. We are a home care company that sends caregivers and nurses to homes to take care of the elderly and those that need it. A lot of our clients go into rehab facilities in the suburbs. It wasn’t until recently that I heard a lot of good reviews coming from Bella Terra Streamwood. I took it upon myself to go into the facility and I saw that they have reinvested heavily in the facility. The new head social worker Agnes is so empathetic and responsive. I cannot say enough how pleasantly surprised I have been from the care coming out of this facility. Top notch! Way to go!

If i could leave this place zero i would my grandma is dying and was placed here, this place is disgusting there’s no one in site. No one checks on her i couldnt come for a few days the same drink was on her tray garbage still no bags they didn’t open her flowers that have been sent . This place should b closed. The staff are probably working too death and don’t blame them. Shame on who ever runs this. This is America where u can put ur dog down for suffering but the elderly get thrown in a place like this and forgot about.

A friend of mine went there for rehab right before Christmas. While there her food came to her room cold and was told she would have better luck getting warm food if she went to the cafeteria. They also kept trying to get her to sign long term care paperwork and were surprised when she said that she was there for rehab. The PT department never worked on what she needed to work on even after receiving an doctors order. They also told her that her insurance, which is a Medicare advantage, had declined to pay for further stay but she could appeal it but had till noon the next day to call. They also blamed a computer glitch for her insurance dening the claim after accepting it.

I've visited many nursing homes during my professional career in the medical equipment sector. I never anticipated becoming a resident at one at a relatively young age. I believe Bella Terra has earned the title of a great rehabilitation center. When I arrived a year and a half ago I was in really bad shape. Bella Terra has proven beyond a doubt that they truly care about it's residents. There's always going to be some issues at any facility. However, I feel that they have done an outstanding job in an unforgiving economy. Special recognition to Maria and Sandra 1st floor for always doing an amazing job. Second floor, Nancy, Murna, Soni, and Jackie , for nurturing me back to health. Jessica, Ivan, and Tashandi in activities for always spreading cheer to all the residents. Special thanks to Edith in administration for keeping it all together. I would recommend this facility to family and friends.Sincerely,C. Wallace

My wife died, she is only 52 years old and was at Bella Terra for two weeks prior. She got sepsis and had a second stroke while she was there. I came in from work on Dec 26 to see how she was doing. She was at Bella Terra just to get sub acute PT and ot, because she was weak from a recent hospital visit, but on Dec 26 she was confused and scared. It turned out she had sepsis. Yet she still had dyslexia and even though the dyalisis nurse saw that Donna was confused, she still had her dyalisis. My wife would go to the hospital that night and pass away a week after New Years. On New Years Eve we found out she had a second stroke. Bella Terra is horrible. 22 min pt, people yelling for help for more then an hour. My wife was 52 and Bella Terra helped lead to her death. Dont ever send anyone here. Its a miserable bottom-feeder.

My dad has been a resident on the 3 rd floor and I just recently placed my Mom on the same unit. The staff has been great on communicating with my family and I, updating us with all concerns and doing the best they can to provide the best care. I appreciate all their hard work and dedication to patient care . Thank you Anne, Tune, Joseph, Rose, May and everyone who has participated in the care of my parents. The team has been great and professional.

This facility is has the most wonderful staff! They are caring and compassionate. They are fueled with a passion that inspires you! Their nurses and CNAs are so hard working. And the other ancillary staff are so easy to work and communicate with!

Worse place ever to get updates on your family member. I even leave messages, can't leave messages for nurses they don't return calls.
