This is day 5 for my father in the rehab section. No complaints about the overall complete assessment from all the therapies and nursing staff, but they definitely have a lack of adequate staff for day to day care for the amount of patients they have. My dad is 92, still very mentally aware, but bed bound with a diaper. When I arrived at 11:45 am one day, he had been in his wet diaper since before breakfast and had been calling for someone for two hours. All his bedding and his stocking were completely wet and I had to help the CNA change everything due to his physical restrictions.They have him fill out his menu choices 4 days in advance and not one of his meals has come correct. They either forget things or totally substitute things that he never asked for. Yesterday he asked for a chef’s salad and soup of the day for lunch. He got lasagna, a breadstick, and a Caesar salad. They hardly ever put the drink he asks for on it. Some staff have been indignant with us when we questioned about the meal. They just kept saying we needed to talk to the kitchen. I had to physically go down to the kitchen myself to ask. Why have them fill out a menu if you aren’t going to have or give them what they ordered?After reading some of the other reviews, we’re having many of the same issues that Kay had below.

Good luck if your loved one needs to be on a consistent medication schedule. I don't recommend this place for physical therapy and even less as a resident.

I was in the facility for 9 days. The room I had was clean and quiet. My biggest issues were getting my meds the way I should have (and I had to ask more than once by an aide who said they would tell my nurse...never to be visited by my nurse unless I kept buzzing for help..and then told by the nurse they were working on them...why wasn't there pre-planning involved here? And sometime an hour or more late...my meds should have been coinciding with one another yet they were even discontinued a few nights because I was told the doctor in the facility stopped them accidentally and then couldn't restart them without my prescribing doctors re-instating the prescription... because they were opioids. I suffered more than one night because of their errors. I was suppose to be able to select my own meals which didn't happen for 6 days after arrival...so got what they had and usually with no condiments of any kind to match my meal. And lastly I was to have a bath/shower on Wednesday and Saturday. I arrived on 12/1...Saturday came and went, so did the following Wednesday...no show. My daughter and I worked together on Thursday to get me showered and shampooed. Towards the end of the shower, an aide came iIn and was a great help to getting me dried off. Someone came on the day I was being released to offer a shower( I refused and had to sign a form stating that) where were you to give me a shower on my days before that? I was also told that if they didn't have time, then I wouldn't get one. I did have some good Physical Therapy while there and tried very hard to reach or exceed the goals so I could be released...sometimes they showed up before breakfast which I never knew when would be delivered...and I would ask the therapist if I could wait to eat first and get a pain pill in me...which caused some upset because they had a lot of others in need as well...I couldn't perform at my best without food in me. The head of the kitchen came to see me after a week of cold food told me how short handed the kitchen was and apologized profusely for that. I told him most all of my meals were a mix of cold and luke warm foods I received and absolutely no condiments...he said once again how short handed the kitchen was and uneducated in the field of food that some staff were...when I did get my food and there were no condiments I would ask for them. The aide said they would be right back with them only never to see them again...after a few days of this, my daughter brought me condiments from you babe am so grateful I got released when I did...thinking about what I tolerated makes my blood pressure rise. I would have a very hard time recommending this facility to anyone.

THEY CANNOT DENY CARE IF YOU DO NOT SIGN THIS DOCUMENT!OUR FAMILY WILL ALL BE FILING COMPLAINTS WITH EVERY STATE HEALTH AGENCY and the INDIANA ATTY GENERAL’s OFFICE.DO NOT LET CARDONE or the Executive Director pressure you or your family members into signing this ILLEGAL DOCUMENT!

The Rehab was remarkable! My 90 year old Mother was there for only 14 days and made tremendous progress! The staff was caring and so helpful. After surgery to repair her femur, we had no comprehension how quickly she would come to our home, but loved this reality. We are ever grateful for the care she recieved.

There are not enough words to express how absolutely wonderful our experience with Hamilton Trace has been. We were introduced to this community late last year as a loved one with some specific needs was needing to move into Assisted Living. We first worked with Rachel Z. (who is a rock star in our book!) and she helped get everything arranged, explained the process, and helped us navigate every step along the way. She did all this with great compassion and patience. The apartments are beautiful and very homey. And most important, the care of their residents is unparalleled. The employees seem to be happy to work there and that translates to wonderful care of loved ones.When an urgent need came up recently for a place to move another loved one to, the staff at Hamilton Trace moved heaven and earth to be sure we could move them there so our family could be together. We will be forever grateful for what the staff at Hamilton Trace did for us. Along the way we have gotten to know other staff who are just as kind, caring, professional, and skilled as Rachel (Caprice, Todd, Sandy, Steven, Emily, the physical therapy team, the maintenance staff, the dining room staff, all the wonderful CNAs, and many others).

Dir of nursing rob refused me medical treatment. They said I would ama but if was funny was admitted and still in hospital .

I really recommend this place! The HR department was beyond efficient and was extremely kind. Their acknowledgment and ease of communication for the minuscule amount of small issues that I did have, and did henceforth resolve, was uncanny.

I love working at Hamilton Trace. I am proud of our beautiful campus and amazing staff who provide great care to all of our residents. We truly care about our residents and families!

My father was at this location. At first it seemed great but after only 2 weeks the doctor said he was ready to come back home he couldn't even use the bathroom safely yet nor had he been given a bath or anything ..... This is not safe. What is the point of going to rehab when you do not get "rehabilitated" how disappointing it is to see how the seniors are treated.

zero stars if i could. Family friend in rehab at this facility. Day shift was great. Night shift sucks. Family friend sat in urine overnight. red light was on, staff was notified. no one came. 845am staff stuck their head in the room and walked away. PLEASE DO NOT SEND ANYONE HERE.

I have been at Hamilton Trace for just over a year. This is the best building I've worked at by far! The staff is caring & compationate, the building is clean & well maintained and our residents are happy & receive great care!! I'm happy to be part of the team!!
