First of all I want to say the staff was very nice and was very responsive to our needs. I was the one who lost my temper, when they asked why my husband was leaving after only spending one night there. My husband went there after being in the hospital due to complications with flu that severely affected his lungs. The room they put my husband in was absolutely terrible. It looked like it was under renovation. Holes in the wall. Paint missing. Floors looked dirty. While the window air conditioner worked it looked like it was falling apart. I felt so bad even leaving my husband there for 1 night, but he needed oxygen to get home, which I brought the next morning. The owners of this place should be ashamed of themselves for subjecting people to this terrible situation, who don’t want to be there to begin with. They should also be ashamed of themselves for allowing this place to become so run down for the amount of money I am sure they are charging people. Just painting the rooms would be an improvement. I have heard that rehab is great there, but you need to get past the living conditions which we just could not do. The pictures I took do not totally reflect the condition of this room. However as you can see my pictures look nothing like the ones they advertise on their website. I did apologize for losing my temper as bad as I did as I know it is not the staff’s fault for the condition of this place.

If you want your loved one to have rights and decency don’t send them. They should be criminally charged for the things that go on it’s definitely against patients rights and I’m sure against whatever moral code they all hold. The fact that it’s went on for a decade from the looks of these complaints are even the Director has worked there the entire time imagine that. The social worker doesn't put together Care plans or anything and they could care less how competent someone is when they give them paperwork to avoid family members.

The facility its self could use some updates and maintenance, but the staff here was amazing! I did not have a bad experience with any of them in my time. Nurse Kim called me about everything! I felt her love for her patients in her voice and even more in her arms when my loved ones final hours came. I am thankful to have had her in this time, when I didn’t know what to do and I felt defeated, she walked me through and got me on the right path. I am very grateful for her time and consideration.

The ONLY reason I gave this place one star is because I had to in order to leave a review. This place is not fit for rodents let alone a human being. My brother went here for rehabilitation after a hospital stay. He declined within 30 days because they did nothing that they agreed to do when he left the hospital. He had pneumonia twice that we his family took notice of and took him to the emergency room ourselves. Embassy was upset when we questioned them about his oxygen and why he was so drowsy. They said he was fine just tired. He was grasping for air even with oxygen on. While finding a new facility for him to go to he had to go back there after his second hospital stay with pneumonia we walked in to visit him to find him slumped over in a wheel chair with no oxygen left in his tank. It pains me to even think about what would have happened if we did not find him ourselves. On another occasion they left him at dialysis his nurse aide the one and only person who was at that facility that cared questioned where he was when she started her shift. Hours later we were called from a hospital saying that he was at the hospital because Embassy of canals Winchester transportation department never want to go pick him back up from dialysis. It is so bad there that we as a family had to come up with the schedule to make sure that family went there several times a day to take care of him. We reached out to the director the administrative staff the charge nurse all these different people who are in leadership at this facility no one ever returned our phone call. We had to leave our job go sit and wait to talk to someone. When we were trying to get him transferred to a different facility Embassy of canal Winchester would not send his paperwork then finally started sending one page at a time. His daughter went there and sat for 4 hours she was told that they were in a meeting all that time. She asked to speak to Lauren specifically and was told she wasn't there. After 4 hours my niece pulled to the end of the parking lot and out walks Lauren. They FINALLY agreed to send all the papers at that point since my niece caught them. I pray for the residents that are still there. This place needs to be closed down ASAP!!

This is one of the dirtiest care facilities I’ve seen. They constantly degrade their residents telling them no one will come get them and they won’t regain strength. If you love your family member please don’t put them here ..

Report everything you see illegal, immoral, incorrect, unsanitary, neglectful and apathetic behavior to the Ohio Department of Health. The list of problems is too long to list. They need shut down.

The facility is dirty. There are more psyc patients than there are recovering patients. The aids yell at the residents and treat them like children, or dogs really. I would not recommend sending anyone you love there. While I was visiting a loved one more than one patient was trying to get out because they hated it there. I know this because they were yelling it loudly and I was trying to visit with my loved one. Very sad. The nurses seemed nice but not skilled. The housekeeping staff was playing around loudly . I overheard and aid say how much she disliked her job...it's just not a place I would recommend.

Family member was there the last 20 days in January of 2022 and they was NOT taken care of and I was there every other day and it was always something they was not doing. And was told by Mount Carmel East that the family member would get 1 hour of PT a day and that was also a LIE. And they didn't keep the wound care clean or changed regularly on this family member and THEY ALMOST KILLED THIS FAMILY MEMBER BECAUSE ON THE 20TH DAY THERE THE FAMILY MEMBER TESTED POSITIVE FOR COVID and they down played it saying everything was fine but I said no way they going to hospital and thank God that happened because they had covid pneumonia and was SEPSIS if embassy would have kept this family member they would have died from the sepsis. And that is what doctor said.DO NOT TAKE YOUR LOVED ONES HERE.

Do NOT send your family members here ! I had a beloved friend here recently who passed away under VERY SUSPISH circumstances !! And to this day his death remains unanswered ! Things were not logged into filing , he was to the point of almost coming home and all of a sudden he is gone ! Staff cannot give an answer as to what happened to him or events leading up to his death !Please if you care anything about your family do not send them here ! We do not want to know another family is going thru what we are !

The place lacks skilled management. Staff knows this and behaves in a unprofessional manner. I have lived here since early February of 2021 after I coded blue twice at Riverside Hospital from COVID-19.

This place is not a happy place as people are screaming "get me out of here" and workers don't seem to be concerned. Sad sad place to be if you are in need of special care of any kind...
