My 90 year old Mom was there for 2 weeks to recuperate from a serious procedure. The Staff were caring, compassionate, and competent. They worked hard to get her ready to go home.

As a new employee of the facility I find it welcoming and warm. The facility is clean and newly renovated. There isn’t any odor like you smell in many other locations. It’s in a great neighborhood. The staff take a lot of pride in the care they provide. I’m really proud to be on the team!

This facility looks as run down on the inside as it does on the outside. Walking in you can tell that the facility is old but that doesn't mean they have to neglect keeping the building up to date they are supposed to be a rehab center but doesn't even have half of the proper equipment to take care of someone in a stroke situation like my step-dad just went through he was just in is facility for a little over 2 weeks and only had 2 showers in total he has had to have certain dietary changes so he is able to eat and they completely ignore those restrictions my step-dad has not been able to walk since his stroke and being at this facility he has had to have multiple weeks worth of laundry had to be taken home and washed at least 6 times which shouldn't have happened because they weren't taking care of his restroom needs all in all this facility is horrible and not worth taking a loved one to th3re is a lot more that issu wrong with this facility

Very disappointed about this place. Patients are not well taking care and bed very uncomfortable. Very depressing place. You should know that it is very sad to put our love ones in a home away from their home, and at least it would be recomforting to know that they are in good care.

I would not recommend this program to my worst enemy. I just spent almost 7 weeks there and the only thing that was good about the stay was interacting with some of the physical and occupational therapists and some of the nurses and techs. Otherwise it was a mess. The food was one of my biggest complaint. Once a week they give you a menu to choose what you want for each meal. Of course, with no pen or anything to write with. Once you get it filled out, they seemed to rarely bring what I ordered, instead many high carb low protein items. I did get some protein but not nearly enough for my dietary requirements, being a person with diabetes. Then they would bring me orange juice, oatmeal, waffles, pancakes. Then they wondered why I was losing weight. The dietician came and said she wants me to be drinking at first 2 protein drinks then escalated that to 3 when I was still losing weight. But they ran out of the Boost drinks which were the highest in protein and lowest in carbs, so they just started giving me Ensure instead which was much lower in protein and much higher in carbs. This would often result in elevated blood glucose which is dangerous when you have a foot wound due to diabetes.Next, hygiene. They have a shower schedule for each person or room number. My days were Tuesday and Thursday. There were 2 weeks I only got showered once. On the day before I discharged (Monday) i asked for a shower because I knew we wouldn't have time that morning of discharge. It did eventually get done but I had to ask several times. So the day I discharged I had to sign a form saying I was refusing a shower. At no point during either of those 2 weeks I only got one shower did anyone even acknowledge that I hadn't gotten a shower the other time I should have. Then, sleep. Every morning between 3 and 4 am a tech would come in and take my vitals. Then the first medication pass was around 6am. Around 7 someone would check my blood glucose and give me insulin and then breakfast is at 8. Physical and occupational therapy would often be after breakfast so sleep was fairly limited unless you go to bed st 8pm.Regarding medical care. When I was discharged from the hospital following a major surgery on my left foot which has caused me to be non weight bearing on my left foot for a minimum of 3 months, my discharge paperwork indicated I would be placed under the care of a doctor at the rehab. I was admitted on a Friday and apparently this doctor comes in every Thursday so I didn't meet him for nearly a week and he basically introduced himself and said he only works there on Thursdays. That was the only time I met this doctor. So the first followup appointment with my surgeon's PA removed my stitches and told me that he wants the pin sites cleaned twice a day. (Look up external fixator to see what they look like.) So this appointment happened on a Friday and the sites were cleaned once on Saturday and once on Sunday, then that was it. At the end of that week a nurse came in and told me they had convinced the Dr that they only needed to clean the sites 3 times a weekWhen I went to the 4 week followup the PA was livid and called the rehab after he'd seen me to confirm daily cleaning. They did somewhat better after that but it wasn't always done on the day shift. All in all this was a very difficult experience to begin with which was exacerbated by the program. I'm home now and very thankful I made it through that without getting an infection which could eventually lead to amputation of my left foot.If you go there be sure to get set up with a meal delivery service like DoorDash, but I wouldn't recommend going there unless you had no other choice.

I went in to interview for the CNA position and had to wait an hour for the lead to call me back for the interview. Walking back to the offices I think I saw 2 OSHA violations and 3 JACO violations. I was told that they would only offer a grave shift even though the posting was for a day shift. The offer said 18-20 dollars an hour and I was told that it was actually 15 but because I was 'so lucky' to be considered for the grave shift they'd bump me up to 16 whole dollars an hour! I'm sorry... If you cant compete with chic fil a and target... you don't deserve to survive as a business in healthcare. At least have the decency to not lie THIS much on your job posting. CNA work is hard and necessary and you basically spit in your CNAs faces with this starvation wage. No wonder your patients hate it there, the people taking care of them are STARVING.

I will just say it up front. The food is atrocious! But, the staff and their care makeup for it 100 times over. I was there for 21 days for physical therapy on my foot. And, I can't say enough about them. I even got a cute haircut in their Salon while there.

This is such a great facility! Everyone has been so kind and friendly. The therapy and nursing teams are so dedicated to providing wonderful care, and the office staff is so helpful, and willing to answer any questions. I would recommend this facility to anyone looking for phenomenal, individualized care for a loved one.

My husband was at Holladay Healthcare for rehab. Everyone was so helpful and pleasant. The staff was very attentive and did a wonderful job. The food was great, just like home made. The therapy team was amazing! His stay was better than expected. I would definitely recommend Holladay to my family or friends!

As someone who has worked closely with skilled nursing facilities for years, what sets Holladay Healthcare Center apart for me is the cleanliness of the building as well as the professionalism and welcoming nature of the staff. The people in leadership roles are very approachable and open to feedback. I also really like the location of the facility.

My mom was there last summer to recover from a heart attack. She told me they never listened to her and never did anything with her. When we were able to discharge her I was told she needed memory care. I started crying as I knew I could not care for my mom. I was told I was hysterical. Sorry I care about my mom.

Holladay Healthcare Center is an amazing place for healing and wellness! Each member of the staff spares no effort nor time to get their patients back to normal life. My mother Knarik was in this center for almost 2 weeks and each day was hard work , effective therapy sessions, love and care from the nurses and staff! We are very grateful and impressed with all you have done for our mother! I highly recommend this place where there’s true rehabilitation for anyone who needs it!Dr. Susie McPhilomy
