I would not recommend this rehab center. When my mom was brought to the building, no one greeted her. The transportation service from the hospital found out her room and took her up and left her. My dad had to help her get settled in her room and into bed. Later someone did come to the room and told her supper would be delivered at 5pm. At 5pm no one came. My mom called at 5:30 and no one answered, finally at 6:00PM when she called an orderly came up and was surprised to hear she had nothing to eat! Then he told her they were OUT of food, however he did bring her a sandwich. She decided she did not want to stay in a place where she was "forgotten" and the nurses and cna's were not responsive. When I came to help pick her up the front desk person was on a face time call and never got off. She did help me with getting the room number. When I went upstairs I saw NO ONE, not a soul working up there. There was a cart in the hall piled with dirty dishes and it was now 7:00PM, so supper was 2 hours ago. I took her downstairs and still saw no one. There was no one to let know we were leaving except the gal at the front desk who finally got off the phone. My sister had also called to let them know the care my mom was getting was not what we expected and talked to a nurse who only continued to complain that she was only 1 person and that she couldn't get to everyone and there so many patients! You know what? That is not our problem.. we were given a set of expectations to help take care of my mom w/a broken ankle and unable to get around on her own and this place sadly failed. It was a beautiful site and the rooms were big and nice, but the staff is severely understaffed and I would truly consider another option before going to this place.

I have worked for Oak Park Place since 2019. The facility at Nakoma has great staff and residents. The building is beautiful inside and out, well laid out with private rooms and showers. We offer Skilled nursing & rehab, assisted living and secured Memory Care unit. We also have 5 star rating with CMS.

Our experience is still pretty fresh (about 2 months) but my brother has been well-respected and staff has worked hard to keep him busy with activities in between my visits. He also loves the food! When there was a recent issue, administrator was quick and fair in addressing it. The front-line staff have all been fantastic, friendly, patient, and responsive to my questions and requests. He doesn’t need much assistance with ADLs outside of meds so I don’t have specific feedback on that. Clean and homey setting.

I hadn't shared my family's experience with Oak Park Place Nakoma until now, but after realizing that several recent extremely positive reviews of Oak Park Place Nakoma are from people who have given very similar, extremely positive reviews of Oak Park Place Baraboo, I felt motivated to offer an actual (and not promotional) review. I reported those other reviews to Google.It's extremely stressful to find a residential care facility for your loved one, especially when you're looking for a place that provides quality assisted care and/or memory care and it's upsetting to me that a care facility would try to improve its Google review score by less than ethical means. We moved my father into Oak Park Place Nakoma in the early days of Covid (July 2020) and moved him out by the beginning of September 2020. There were some definite positives to his very short experience there - some individual staff were amazing, the physical facilities were beautiful, the food was great (really good). But the overall care was not what we were hoping for, including communication issues between staff about medication administration. This was exacerbated by staffing issues they were having among senior, leadership positions: during the short amount of time he was there, they let the Director of Nursing go and had some other turnover in Administrative positions. Although it was very stressful to move him out after just having moved him in there, I didn't feel secure leaving him there. I wish the staff at Oak Park Place Nakoma the best - as I said, a number of individual staff were really caring, kind, and good at their job. But the overall experience did not go smoothly.

Very nice facility with extremely knowledgeable and friendly staff. You can most definitely trust the care you receive, as well as getting the answers you need if you’re questioning if Oak Park is the place for you. Michelle was so nice and accommodating answering our questions!

The staff is very welcoming and nice, facility is also very clean. You won't be disappointed if you put your loved one in the Oak Park Place Nakoma.

My Dad was in the memory care unit for about 2 months. We could not get him out soon enough. We worked with the staff and the administration on a regular basis with no help or change. The care he was given I would say was neglect. He would be sitting in urine soaked clothing on several occasions on a daily basis. Not only through his clothing but through furniture cushions and bedding. He was on a 2 hour care plan, that did not happen. Many times staff would come in to change him and after about a minute of trying would say "he won't let us" and would leave. One time we walked in and Dad was sitting on the loveseat with his pants half down soaked in urine like they just left him halfway through. My dad was not able to pull his own pants down. They did not do daily care, very rarely oral care, they say they attempted showers, but how much can you really say you tried in 2 minutes before you come out and say “he wouldn’t let me”. We went in at 11:30 once and he was laying in bed in urine, hadn’t even been offered breakfast yet. We have no proof that meals were missed, but this might make you think maybe! This is not the staff’s fault, it is an administration issue. They know they are short staff and that the staff is not trained adequately if at all, and they just keep on bringing more residents in, keep the money coming even if we are not doing what we promise on admission. We met with administration on a weekly basis, we worked with them, we tried. They gave us a bunch of talk and knew what to say, but there was no action to correct anything. Yes, I say neglect. I write this review in hopes that nobody else’s loved one and family have to live through this hell even for a short time. I feel so bad for the residents who live in this memory care unit that have nobody to visit and are not able to express to anyone how they are or are not being cared for. This looks like a beautiful place, but looks are not what matters in this situation.

Our experience at this facility was unbelievable bad. From unaccountable administration/ownership, to outright neglect of residents, to lies and unkept promises to "use this as a training opportunity" this facility should be crossed off your list of options. Staff was overworked and poorly trained, with few truly caring about the residents. Facility administration kept saying the right things, but never followed through or held anyone accountable. Ownership, who would not reply to any concerns, and who "wanted to build a place his family could live in" must not like his family.

I have attended quite a few events at Nakoma prior to Covid, They have been well planned and educational. I have been fortunate to be invited to the virtual meetings and the last one on Mindfulness was quite helpful at these times. The community has always been clean and welcoming and I look forward to future events. I have a friend who's husband lives there and the staff have been very good and getting her means to visit with him and while not ideal it has given her peace.

Oak Park Place Nakoma is a great Senior Living facility. My uncle was on the memory care unit here , my family and I were thrilled with the staff we worked with, a special shoutout to Lavenia for always going above and beyond for him. We were very happy with the care he received up to his passing.

For the rehab part. I give it a one for the people who really cared there. And the food was good. Physical therapy was the best we ever had!! Place was new and nice. But there were days I would come in right before lunch and she had not been helped get ready for the day, soiled and in the same clothes from the day before. The floor had food spills on it from the 1st week we were there until the last week. And she was there for 60 days. Pills were not administered correctly. Call button was not attended to in a timely matter and again soiled because of it. Lunch food was still in the room at dinner time. Under staffed!! Lost several employees when we were tgere. Didn't bathe her even if it was scheduled. I had to ask several times.

Staff is great. However, they are understaffed. It took 4 weeks to get my MIL's bathroom cleaned! CNAs should not be doing cleaning especially when it takes the MedPass 4 hours to do her med set up and delivery. MUCH admin turnover even since middle of October. Food is good. Residents have demanded the owner be present at meetings. A nurse said it would not happen. We shall see. MIL is 83 and can toilet and bathe herself. Again, all the nursing staff is great! They are understaffed, though.

Oak Park Place - Nakoma is generously offering its beautiful facility on Sunday mornings for our congregation from Prairie Unitarian Universalist Society to meet most weeks. We have been gathering there most Sundays since August 2019 and we will continue to through the late Spring as our building's remodeling finishes up.
