In fairness to this practice, I have been going here 20+ years and I haven’t had that many issues that would compel me to take time out of my life to write a review but I had a disturbing experience 9/19/25 with the phlebotomist that deserves to be addressed and I know writing my experience in a typical non public secret survey wont do. It needed to be addressed here where maybe someone would care about their Google review. In my many decades of living, Ive never experienced such a bad blood drawing experience! The lady brought a flimsy pillow in the room, told me to lay my arm straight on it which was impossible considering the angle of my chair, her chair and the cushioning and wobbly of the pillow. She stuck the needle in my arm and because there was no hard surface underneath my arm to stabilize it, the needle fumbled to a point where blood started skeeting profusely out my arm all over the pillow and onto the floor and probably other surfaces in the room. The way the blood was pooling out of my arm, I thought she’d broken the needle in my arm! It was a very scary and highly unpleasant experience! When we finally got the blood to stop pouring from my arm, I asked her why aren’t we in the lab so i can put my arm on a stable arm rest surface to keep it positioned correctly and stable. She said they always use pillows there which is absolutely not true because I’ve been coming to that office for over 20 yrs (much longer than she’s been working there) and I have never ever had any phlebotomist to use a freakin pillow to try to stabilize my arm to collect blood. NEVER! In fact, I’ve gotten blood drawn for decades at various places and that was my first time ever seeing someone using an unstable wobbly pillow as a arm landing or arm rest to draw blood. Very unprofessional and unsafe! I told her she can’t take blood from my arm again and we have to into the lab so I can lay my hand on a firm stable surface (ie the arm rest) and she can take the blood from my hand instead of going back into my arm again. That was done and thankfully there were no traumatic issues with that process. I understand we’re in a new age but some things should never change and drawing blood from a stable surface is one of them. This is the way it’s always been safely done and should be continued to be done. That employee needs to refrain from trying to draw blood in such an unsafe, unstable, unconventional and unprofessional manner.

The lady at the front desk with the bob is one of the worst front desk employees I have ever seen. No type of professionalism besides saying “ma’am”. She was beyond rude hardly communicating with me when it came to my questions. After my first experience with her I made an effort not to even engage with her because the other front desk lady is honestly so sweet but her co worker is truly not cut out for the job. I will probably be switching entirely because of this. But first I will be contacted HR because the front desk people are the first you see when you walk into the office. People are already sick or injured or both and to have such a terrible experience with this lady when I was already anxious was the cherry on top.

Bukola Famakinwa is an Arrogant, cruel hearted Angry and aggressive provider that not only is verbally aggressive to her patients but also has no, genuine will of caring for patients. Third time seeing her and today and every time she blames my physical symptoms On my Mental diagnosis. She really thinks that I am that ignorant to the point where I don’t know that she is insulting me. Today I came into the cascade office with symptoms of back pain and frequent urination and headaches and a little fatigue at times. Her, exhibiting aggressive energy with her words and visual aggravation by me through her body language, told me that all my symptoms were related to my mental diagnosis that I have had for over 10 years. I’m not ashamed of having a mental health disorder. What I am ashamed of is trusting her to be a professional provider and putting money in her pocket over three times coming to her for care. As a patient, seeking courtesy and support and every time getting a African Provider(her) Attempting to antagonize me and patronize me like I don’t know that my physical symptoms has absolutely nothing to do with my mental health diagnosis. I will never ever ever put money in her pocket again. As a Black American born woman, I will take my business elsewhere to a provider that is black American and at least acts like they are truly there to help me. She does not like my Kind and she shows it with her nasty and aggressive behavior towards me. For that reason I am done with the Cascade Piedmont Facility Indefinitely

I will not see Dr. bernardin again, I was recommended to her and my first experience was good but after my last visit it took her a whole month to put my results in my my chart and I still have to come back to re-do my pap because she didn't do it right the first time so I switched providers but never again.Then she cancelled my follow up to redo my pap as well

I'm not a big fan of going to doctors. My wife was able to talk me into going after a deal we made. The only thing I can say is Dr. David Moise is TRASH!!! I can even say if he did his job and did it effectively because he canceled on me TWICE. The first time I get it something may came up but the explanation was because he had schedule conflicts. which I understand but, how do you do it twice having schedule conflicts. I didnt even wanna waste my time a third time so I decided to go to another Dr. Hopefully people who read this avoid David Moise at all cost AKA the conflicting scheduler.

I see why this office Google reviews are so low. If this place wasn't convenient to my home they would not get any of my business. Had to go there on 3 separate occasions to get blood work and urine sample. No fault of mine they are just incompetent. Sad!!!!

I was originally a patient of Tonya Knowles, NP and had a wonderful experience. Unfortunately she is no longer with this office so I made an appointment with Janice Thomas, NP. I wish I could say my experience with her was a good. I’m a nurse and I’m very informed about my healthcare. She mentioned ordering a lab that I felt was not needed. I declined the lab however she preceded to order it without my consent. It has also been awful getting refills on my prescriptions. Replies to my messages take 4-5 days. She also cancelled a skin prescription that I’ve been on for years with no explanation and will not reply when I ask why. I will not be returning back to she this provider. The office staff are very nice and the wait is always short. I’m hoping to find a better fitted provider at this location.

The front desk staff is always nice and friendly. I'm seen usually waiting 15 to 20 minutes of my scheduled appt time. When blood work has to be done the nurse is quick and efficient here, one of the few who can draw my blood with only one stick. Dr. Bernadin listens to my issues and always provide the appropriate help needed.

I have been to this office about 3 times. They are always really nice when you check-in and I never wait a long time to be seen. My mom recommended her Doc which is Dr Bernardin. The first time I made an appt (2022) I couldn't get her she was booked, so I had to settle for the nurse practitioner which was cool. The NP was great and very informative. So I made my next yearly appt for Dr Bernardin before I left for the following year, which was this year. Just had my appt with her and she was really nice.There was just one thing I am not to fond of, one being that once the Dr leaves after she has met with you, you don't have a chance to see her again. For instance I thought I needed a pap so she stepped out so that I could prepare and she could inform the nurse to assist her. However once I realized that I didn't need the pap b/c I had actually had one earlier in the year and had forgotten, I informed the nurse. She then told Dr B, however, I had some follow-up questions, but I was told that if she came back into the room to speak with me that I would be charged...???. I was confused by that. So the nurse said she would relay my concerns. And that just made me feel as though we are only $$ signs to them and not actual patients. Although while she is in your presence she is very attentative, just make sure you get out all you want to say or ask, b/c once she leaves there is no return unless she is going to do some sort of exam.If you make an appt with Dr B, DO NOT CANCEL or it will be hard to get another appt. And for me I like to stick to 1 Dr so that I can get a report with them.

The check in always great. When you call they are nice but you have the hardest time on refills. I feel like I will stroke out waiting on HBP meds. Like my blood pressure be too high to playing with and it's not manageable yet. Scheduling is off. Never miss your appointment at this clinic or you will regret it deeply

Trying to get my HBP meds refill is like pulling teeth. My Dr. is no longer there, my NP is no longer there. I was never contacted about their departures... and I can't get a refill without an appt (in 6wks). The front desk person (I wish I knew her name) though it was funny and just repeated... you need to make an appt before you can get your meds. Unacceptable, unprofessional and unbecoming to the profession of "healthcare". If you care about your health, go elsewhere.

If you care about your health... go elsewhere. They are making money off refill appt. I don't need an appt for a refill. My med records are there... in the office. So sad. The front desk person is awful and rude.

I went in today for my annual physical. Ms. Tammy greeted me with the most pleasant attitude. She was engaging and informative. Dr. Bernardin is my doctor and she is also very pleasant. I am very pleased with this location.
