“I was referred here when I was in search of a new primary care physician as I had medical issues that were dibilating and needed a doctor to care enough to run actual test s, as previous Dr didn't. I have need registered and seen several times over past year by the emergency PA(He was Great) and once or twice by Dr. Nichols. My wallet was stolen with my DL SS card in it so SSA office said I needed a letter from my doctor verifying my identity. I was told Dr. Nichols refused to write letter so I could not get a new SS card needed to get a new id. Weeks later, I had a scheduled appointment. I showed up and was asked for my id(was never asked before). I stated didn't have one due to Dr refusal to write letter. I was turned away and told couldn't be seen. Why all of sudden after me bringing up id was I being asked for one after office refused to provide letter. Better question my attorney and I have is why was this not required and asked for at initial New patient intake? Why am I being punished for the fact the office staff don't know their job or didn't do it. N”
