I was sent an invoice for a bill that was put into collections, however I have yet to receive any documentation of services rendered and the email they have does not appear to work. I’m not sure what kind of operation this “lab”is running but it is very suspicious.

This place is a nightmare. I'm extremely upset at my doctor's office for using their services. BCBS has reached out to them back in the beginning of September, to this date they have not fixed the issue.

slow, unhelpful, rude. all of the staff that was actually GOOD at their job had the sense to leave and everyone else is not useful. If you have the option to utilize another "lab" please do.
