I was going through some older papers and found this I don't Recognising it before and the address I never lived there what should I do

This is a scam and fraudulent company. Their phone number is fake. I recieved a pre-approval letter from them and they got a $37 check for annual membership fee to send me a credit card. But all BS . I'll report it to the police and my bank.
